We proudly provide a range of home care services and are available 24/7 to help when you or a loved one need it most.
After working in the banking industry for over 25 years, I decided to make a career change and focus on an area that has profoundly impacted my life. Both my mom and father-in-law faced situations where they needed extra care and weren’t able to live in their own homes unassisted. I became aware first-hand of the emotional and logistical challenges that this need places on a family. I founded Home Care Connectors (HCC) because of a strong personal belief that the people we love deserve our best in their time of need. Knowing that your loved one is cared for in the best possible way is one of the most important decisions one can make.
Together with a team of experienced healthcare professionals, we have proudly established a company, a culture, and a standard of service that is recognized for excellence by our grateful clients. Being entrusted with the care of someone in need is truly one of the greatest responsibilities, and when done with distinction, it is also one of the most fulfilling.
In addition to my personal journey with aging loved ones and the experiences of being a Home Care Director, I am a Certified Dementia Practitioner (CDP), have attained the Certified Senior Advisor (CSA) designation, the Alzheimer’s Association’s dementia care training certification (essentiALZ.org) and am a certified Medicare counselor with the CT State Department on Aging (CHOICES) – all with the aim of sharing an empathetic, educated, and valuable perspective with the clients and the families we serve. I have lived in Stamford and Greenwich CT with my wife Sarah and our four children for over twenty years.
With over twenty years of nursing experience I have had the great opportunity to work in many diverse fields: emergency, cardiovascular step-down, pediatrics and oncology. I have over ten years of experience in the home care setting, as well as teaching Nursing at the University level. This experience has given me the ability and advantage to think outside the box and grow. I get to make a profound difference every single day.
One of the most rewarding aspects of a career in nursing is the ability to connect with our patients on a uniquely personal level. I became a nurse so that I could have an impact on the lives of others and have a career that makes a difference. I have never had a regret about my career choice. While I often meet my clients under very difficult circumstances, I get to know my patients very quickly and have the opportunity to play an
important role in their lives.
During my free time, I enjoy running, watching movies and vacationing with my family.
My career as a nurse has led me down multiple paths. These different experiences have made me more prepared and equipped to deal with a vast variety of diverse and complex nursing needs. My acute care experience includes Neurologic and Orthopedic, Medical Oncology, and Telemetry/Cardiac. I was a clinical nurse in the Medical Intensive Care Unit and I am BLS (Basic Life Support) and Advance Cardiac Life Support (ACLS) certified. Part of my responsibility as a Nursing Administrator is to respond and participate in codes and rapid response at the hospital. My long-term care experience includes Charge Nurse of a rehab unit, Nursing Supervisor, Assistant Director, and Wound Care Specialist. As a wound care specialist, I rounded weekly on all patients with wounds, assessed and monitored, and made recommendations
for treatment.
I have over twenty years of nursing experience. I have been a part of the clinical and administrative team in both long term care and acute care settings. My most recent experience is part of the Yale New Haven System for eleven years. My experience transcended from a MICU/SDU Registered Nurse at Yale New Haven Hospital to a Nursing Administrator at Greenwich Hospital, a subsidiary of Yale New Haven. My educational background includes a Bachelor of Arts in Economics, Registered Nurse (RN) and a Masters
in Nursing Administration.
I am very excited to be part of a professional health care team that embodies the core values of nursing. At Health Care Connectors we strive to sustain our patient’s autonomy and dignity. Our care is focused on the needs of the patients and their family. The support we provide encompasses the physical, emotional, social, and spiritual well-being of our clients.
I have three children who are a source of boundless joy in my life. I enjoy yoga, a good work out, and spending time
with family and friends.
I have worked in the medical profession for more than 25 years. My most recent position has been head nurse at the Greenwich Hospital Oncology Unit, where I’ve worked for the
past eight years.
Throughout my career, I’ve gained a range of experience, doing everything from leading the preparation of an outpatient surgical center for sale and state certification to day-to-day oncology nursing duties. I’m not only a registered nurse, but also have a Bachelor of Arts degree in international relations, as well as OASIS training and both BCLS and ACLS certifications. I also have expertise in the areas of palliative hospice care, where I’m Yale Hospital certified; speech coaching; bio-chemotherapy (certified); and hospice. My hospice experience includes admissions, planning, and training of aides.
I joined the team at Home Care Connectors because I believe my skills are a perfect match with HCC’s mission. I seek to keep loved ones safe in their respective homes with dignity and
grace, using a holistic approach.
I live in Greenwich with my three sons. During my spare time, I enjoy CrossFit, running, and competing in triathlons.
As the Client Care Manager at Home Care Connectors, my primary responsibility is to align the right caregiver for each client’s customized plan of care. I draw upon my 15 years as a Certified Nursing Assistant (CNA) to evaluate the clinical skills and experiences of every home health aide hired by Home Care Connectors, and I pay special attention to appreciating the each caregiver’s unique personality and demeanor. My goal is for our clients to be delighted with our service, not only from a clinical perspective, but as importantly from a personality compatibility viewpoint. I am proud of the reviews our clients share about our services on Google, but even more proud of the heartfelt calls I receive from clients and loved ones when they share the positive impact our team has had on the client’s quality of living.
I believe excellent home care is the proper combination of healthcare and hospitality. Growing up in Santo Domingo in the Dominican Republic, the daughter of a physician, I came to recognize the value of excellent medical attention and the appreciation families have of such support in their time of need. Previous to my healthcare experience, I had worked in careers in the service sector, where exceptional client service and concierge level attentiveness were the positive differentiators. In my role of Care Coordinator, I draw upon those experiences to ensure our team of home health aides provide the highest level of care and companionship to our clients and the families entrusting us.
I have lived in both Greenwich and Stamford CT for 18 years with my children Elina and Emmanual. I am an avid animal lover and enjoy taking care of my dog, Cooper.
I have 11 years of impactful direct patient care experience. My greatest passion is to bring healing and comfort to my patients and their families in challenging times.
As Care Manager, my responsibilities include helping our clients and their families find healthy perceptions of themselves and strengthening their relationship with our caregivers.
My educational background includes a Bachelor of Arts degree in Political Science. I am also trained as a Certified Patient Care Associate, Patient Cardiac Monitor Tech, and Patient
Safety Video Monitoring.
I grew up in Ghana, was educated in London and now live and work in the Greenwich area. My children attend Brunswick School in Greenwich. In my free time, I like to work on cars and spend time with my family.
I graduated from college with a degree in finance, and I pursued a successful career in the field of accounting for over 20 years. In all those years of working with numbers, I eventually learned that the most valuable aspects of my career came from communication–talking, listening, and developing connections with people. This amazing discovery led to the most important change in my
walk of life.
Once I switched to the healthcare field, I was able to use the interpersonal and organizational skills that I had developed over the years. This experience has completely reshaped the professional side of my life by helping others face various life
and health challenges.
In my position as Recruiting Manager with Home Care Connectors, my primary goal is to search for dedicated, kindhearted individuals that can take excellent professional care of our clients.
I have lived in Norwalk for over 28 years with my daughter and sister. In my spare time, I teach Financial Literacy in my community. I enjoy reading, exercising, gardening, and listening to
motivational podcasts.
